The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.
About the Campus: Crestwood Champion Healing Center is a Mental Health Rehabilitation Center (MHRC) located in Lompoc, a beautiful Central Coast city in Santa Barbara County, known as “The City of Arts and Flowers.” Champion Healing Center has innovative trauma-informed recovery programs that are based on Crestwood’s commitment to providing a continuum of care that puts persons served with significant mental health issues on the road to recovery.
The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.
Pay Range:
$24.20 - $26.19 USD
