Job summary

National Church Residences is seeking a Service Coordinator for Mabess Manor, a senior apartment community in Campbellsport, WI. You will coordinate activities, connect residents with services, and build community links to support independence.

Part-time role (16-20 hrs/wk), Monday–Friday 8am–5pm, at $24.70/hour with comprehensive benefits. ABachelor’s in social work or related field and 2+ yrs experience with seniors preferred.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two+ years of experience delivering social services to elderly or families preferred.
• Working knowledge of supportive services and local community resources.

Responsibilities

• Advocate, organize, problem-solve, and deliver measurable results for residents.
• Coordinate programs and activities for residents.
• Liaise with community agencies and service providers; seek new services.
• Identify low-cost providers and negotiate discounts when possible.
• Engage residents to identify needs and referrals to services.
• Develop a Resource Directory of local providers and services.
• Sponsor educational events on health care and life skills.
• Provide quality customer service and address resident concerns.
• Assist residents with accessing housing-related services and transportation.
• Monitor and relay resident/family feedback and concerns.
