Job summary

The role involves managing a caseload up to 25, coordinating Program Plans, and serving as an advocate with community agencies. Travel within the Sacramento region may be required to support clients and providers.

Qualifications

• Applicants must possess a Bachelor's or Master's degree in Social Work or a related field; demonstrate excellent written, verbal, and interpersonal communications skills; and have strong time management and organization skills.

Responsibilities

• Provide case management services in the Intensive Case Management Unit.
• Maintain a maximum caseload ratio of 1:25 as required by law.
• Participate in developing and monitoring Program Plans; advocate for clients with community agencies and the criminal justice system.
• Assist co-workers with special projects, coverage, and on-call duties; travel within Sacramento County and surrounding areas as needed.
