Job summary

Burke is seeking a Service Coordinator to provide ongoing direct support to individuals served through General Revenue Programs and the Community First Choice Waiver. You will coordinate services and supports to promote independence and access to resources.

You will travel regularly to individuals' homes, schools, and day service environments to monitor services and ensure needs are met, while maintaining timely documentation and collaboration with families, teams, and providers.

Qualifications

• Bachelor's degree in an accredited field such as Social Work, Rehabilitation, Psychology, or related Human Services field.
• Valid Texas driver's license required.
• Reliable transportation and driving record meeting Burke's insurance requirements.
• Proficient and timely documentation skills.
• Good typing skills and working knowledge of Microsoft Word.
• Strong organizational skills with ability to meet deadlines.
• Ability to complete required trainings (SAMA, CPR, First Aid, Defensive Driving).

Responsibilities

• Complete assessments and required documentation.
• Facilitate program planning and development.
• Monitor services to ensure individual needs are addressed.
• Link and refer individuals to services and community resources.
• Provide crisis prevention and management support.
• Develop understanding of reimbursement practices and program guidelines.
• Communicate with individuals, families/LARs, team members, and resources.
• Coordinate services with internal and external providers to Burke's mission.
• Travel to individuals' homes, schools, and day environments for monitoring.
• Maintain accurate, timely documentation meeting deadlines.
