Job summary

Threshold, Inc. is seeking a Service Coordinator to support our Community Programs in West Bend, Wisconsin. This role coordinates meaningful community activities, helps individuals with disabilities develop life skills, and ensures programs run smoothly.

You will plan outings, support independence and social skills, guide direct service staff, and maintain documentation while collaborating with families and community partners. Associates degree plus two years of related experience are preferred.

Qualifications

• Associate degree and two years of related experience, or an equivalent combination of education and experience.
• Strong organizational, communication, and problem-solving skills.
• Ability to coordinate multiple activities and work collaboratively with others.
• Valid Wisconsin driver's license, good driving record, and access to an adequately insured vehicle.
