Job summary

The Threshold, Inc. in West Bend is seeking an Activity Leader to support adults with disabilities as they build skills, participate in activities, and stay connected to their community.

Schedule is Monday–Friday, roughly 7:30/8:00 AM–3:30/4:00 PM with 35–40 hours per week, located in West Bend, WI. Starting pay is $15+; there are no nights or weekends. You'll guide activities, outings, and personal goals while ensuring safety and engagement.

Qualifications

• High school diploma or equivalent.
• Valid Wisconsin driver’s license with acceptable driving record.
• Ability to safely perform essential functions of the position, with or without reasonable accommodation.
• Willingness to complete required company-provided training within the first six months (Medication Administration, CPR/AED, First Aid, and Non-Violent Physical Crisis Intervention).

Responsibilities

• Assist with on-site activities such as crafts, games, and group activities.
• Help clients participate in community outings and events.
• Support individuals as they work toward personal goals and independence.
• Encourage participation, social interaction, and connections with others.
• Provide assistance with personal needs including grooming, eating, transfers, repositioning, and personal cares.
• Build positive, supportive relationships and model appropriate behavior.
• Transport clients to community activities using agency vans.
• Observe and document client progress, needs, and changes.
• Assist with administering medications or related procedures as trained.
• Maintain a clean, safe program environment.
• Collaborate with team to provide consistent, quality support.

• Observing and documenting client progress, needs, behavioral changes, and other important information.
• Administering medications or assisting with medical procedures as trained and needed.
• Helping maintain a clean, organized, and safe program environment.
• Working closely with other team members to provide consistent, quality support.
