Job summary

University Health is seeking a Service Coordinator-Licensed Vocational Nurse (SC-LVN) to collaborate with medical, behavioral, and social support staff to holistically evaluate STAR+PLUS Members’ needs and provide education and health information to the Member, Representative, and Support Network.

The SC-LVN coordinates LTSS services for STAR+PLUS Members, addressing physical, behavioral, functional, and psychosocial needs while following Community First Health Plans policies and fostering

Qualifications

• Three years of clinical nursing experience preferred.
• Two+ years in a managed care environment or with people with disabilities and chronic/complex conditions preferred.
• Data entry experience required.
• Call center experience preferred.
• Experience with RUG/MDS/PASRR preferred.

Responsibilities

• Collaborate with medical, behavioral, and social support staff to evaluate members' needs and provide education.

POSITION SUMMARY/RESPONSIBILITIES

The Service Coordinator-Licensed Vocational Nurse (SC-LVN) collaborates with medical, behavioral, and social support staff within the Service Coordination team to provide a holistic evaluation of STAR+PLUS Members’ individual dynamics, needs, and preferences while providing education and health-related information to the Member, the Member’s Representative, and the Member’s Support Network. Coordinates STAR +PLUS Members covered and uncovered Long Term Services and Supports (LTSS) services, while meeting the Member's physical, behavioral, functional, and psychosocial needs. Complies with Community First Health Plans, Inc. (Community First) policies, procedures, and protocols for establishing and maintaining good working relationships with co-workers, Members, and guests

EDUCATION/EXPERIENCE

A minimum of three years of clinical nursing experience is preferred.At least two years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions is preferred. Data entry experience is required. Call center experience preferred.Experience with RUG/MDS/PASRR preferred

LICENSURE

Current unrestricted license in the State of Texas as an LVN is required
