Qualifications

Responsibilities

  • Oversee and manage an Early Intervention caseload of up to 50 cases and 75 units weekly.
  • Follow up on cases in a timely manner after assignment.
  • Monitor therapy services (Special Education, Speech, PT, OT, Autism) for multiple children.
  • Assist families in identifying available evaluation or service providers.
  • Work independently to meet paperwork deadlines and NYS/NYC regulations.
  • Educate families about advocacy services and coordinate with healthcare providers.
  • Develop transition plans to preschool or other services when appropriate.
  • Navigate portals and databases, updating information for each child.
  • Conduct home visits or meetings as needed and document daily activity.
  • Attend monthly supervision meetings for training and development.

Job Summary

We are looking to hire a Service Coordinator to join our growing Melville, Long Island team. The Service Coordinator will manage and oversee the process involved in assisting families enrolled in the Early Intervention Program (ages birth to 3). The Service Coordinator combines the traditional case management activities of organizing and coordinating needed services with the philosophy of family-centered care. Building and strengthening relationships is of the utmost importance while adhering to the timelines and procedures set forth by the counties and NYS guidelines.


Responsibilities


  • Oversight and management of an Early Intervention caseload of up to 50cases and 75units weekly minimum.

  • Service Coordinators are paid$11.50 per unit and units are time based

  • Timely follow-up on cases upon assignment.

  • Monitor ongoing services and therapist performance for a number of children including Special Education, Speech Therapy, Physical Therapy, Occupational Therapy and Autism Services.

  • Assisting families in identifying available evaluation or service providers.

  • Work independently and meet the timeliness of all paperwork submissions and implementation of NYS and NYC Regulations and Policy.

  • Educating families about advocacy services.

  • Coordinating services with medical and health care providers.

  • Facilitating the development of a transition plan to the preschool and other services when appropriate.

  • Navigates through web-based portals and customized database independently with minimal supervision, by updating and uploading necessary information and documents for every child.

  • Conducts field work with the families as needed such as home visits, office meetings at regional or public locations upon family request.

  • Documents and submits all activity on a daily basis with accurate information and meets the unit requirement.

  • Attend in-person monthly Supervision meetings for training and continuous development.

  • Other duties as assigned by manager.

Physical Requirements


  • Occasionally ascends/descends stairs going to, while at, or leaving work location or facilities traveled to for work events.

  • Occasionally moving and/or lifting objects at work weighing up to 20 pounds such as files.

  • Must be able to remain in a stationary position for prolonged periods of time up to 90% of the work day working on a computer/electronic device.

  • The ability to observe details at close range (within a few feet of the observer).

  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.


Compensation

FFS$46per hour /Paid Training
