Job summary

VersAbility Resources, based in Hampton, VA, seeks a Service Coordinator to guide families through the Early Intervention process, from intake to IFSP development, ongoing coordination, and transition planning.

Role Overview

VersAbility Resources is seeking a compassionate, organized, and family-focused Service Coordinator to join our EPIC Early Intervention team. This position plays an important role in helping eligible infants and toddlers and their families understand, access, and coordinate the services and resources they need to support positive developmental outcomes.

As a Service Coordinator, you will guide families throughout the Early Intervention process-from intake and eligibility through Individualized Family Service Plan (IFSP) development, ongoing service coordination, and transition planning. You will work closely with families, therapists, medical professionals, childcare providers, community organizations, and other partners to help ensure services are coordinated, timely, and responsive to each family's priorities and needs.

This opportunity is well suited for someone with a background or interest in early childhood development, social work, human services, nursing, special education, psychology, child and family studies, or related fields who enjoys building relationships with families while managing detailed case coordination and documentation.

What You'll Do
  • Coordinate Early Intervention services for assigned infants, toddlers, and their families.
  • Complete intake, screenings, eligibility activities, service-planning assessments, IFSP development and reviews, and discharge activities.
  • Facilitate IFSP meetings and work with families and interdisciplinary team members to develop meaningful, family-centered goals and outcomes.
  • Monitor services and IFSPs to help ensure families receive appropriate and timely Early Intervention support.
  • Serve as a liaison among families, therapists, physicians, nurses, childcare providers, community organizations, and other service agencies.
  • Help families identify and access community programs, resources, support services, and other available assistance.
  • Support families in understanding Early Intervention services, procedural safeguards, available resources, and self-advocacy.
  • Develop and coordinate transition plans as children prepare to transition from Early Intervention services.
  • Maintain ongoing communication with families and members of the child's IFSP team regarding services, progress, needs, and changes.
  • Complete accurate and timely case notes, billing documentation, eligibility records, IFSPs, and other required electronic documentation.
  • Manage multiple families, appointments, meetings, documentation requirements, service needs, and regulatory deadlines.
  • Recognize potential child health, safety, developmental, or family concerns and follow established reporting and referral procedures.
  • Participate in required meetings, training, professional development, and occasional weekend Child Fair activities.
  • Perform other related duties as assigned.
Qualifications - Required

Candidates must meet one of the following Virginia Early Intervention education/experience pathways:

  • Bachelor's degree in an approved Early Intervention-related field, such as allied health, rehabilitation counseling, recreation therapy, occupational or physical therapy, speech-language pathology, child and family studies, counseling, early childhood, early childhood special education, human development, music therapy, nursing, psychology, public health, social work, special education, or another related field or interdisciplinary program approved by the State Lead Agency; OR
  • Associate degree in an applicable related field, such as occupational therapy assistant, physical therapy assistant, or nursing; OR
  • High school diploma/GED or an undergraduate degree in an unrelated field with at least three (3) years/36 months of qualifying full-time experience coordinating direct services to children and families and implementing individual service plans, in accordance with Virginia Early Intervention requirements.
Preferred
  • Previous experience in Early Intervention, service coordination, case management, developmental services, human services, or working with infants, toddlers, children with disabilities, and families.
  • Experience coordinating services, developing or implementing individualized service plans, maintaining case documentation, and connecting children and families with community resources.
Work Environment and Physical Requirements

This position works across a combination of office, family home, childcare, community, and other Early Intervention service environments. Frequent local travel is required to meet with children, families, providers, and community partners and to provide service coordination in children's natural environments.

The position involves regular computer and electronic documentation work as well as frequent interaction with infants, toddlers, families, healthcare professionals, therapists, service providers, and community partners. Employees must be comfortable entering and exiting a vehicle frequently and navigating the varying environments encountered during home and community visits.

The role requires periods of sitting, standing, and walking; occasional bending, stooping, kneeling, crouching, and reaching; and the ability to work at or near floor level when appropriate while interacting with infants and toddlers. Employees may occasionally lift and carry up to 15 pounds, including laptops, files, assessment materials, and other work-related items.

Schedule

This is a full-time position scheduled Monday-Friday from 8:00am-4:00pm. Candidates must be flexible to adjust their schedule based on family availability, meetings, service coordination needs, program activities, and other operational needs. Occasional weekend availability is required to support Child Fair activities when requested.
