Qualifications

  • Bachelor's degree in applied Behavior Analysis, Counseling, Psychology, Social Work or a related field.
  • Valid Texas Driver’s License is required.

For over 50 years, MHMR has provided community-based services for youth and adults with intellectual and developmental disabilities (IDD), mental health conditions, and substance use disorders, as well as babies and young children with developmental delays. Direct Care - Clinical Therapist/Social Work/Case Worker

As a Service Coordinator, you are more than a case manager- you are the person helping individuals turn their goals into real opportunities and meaningful progress. You will work directly with individuals enrolled in HCS, TxHml, and GR CFC programs, helping them identify their needs, advocate for their preferences and build a life that reflects their personal goals and independence. Every conversation, connection and service you coordinate has the power to create lasting impact.

In this role, you will be the trusted guide ensuring services truly align with each individual’s Person- Directed Plan (PDP). From connecting people to valuable community resources to monitoring and coordinating essential support, you will play a vital role in delivering consumer-driven services that empower individuals and families throughout Tarrant County. Qualifications

Minimum Education: Bachelor's degree in appliedBehavior Analysis, Behavioral Services field, CommunicationDisorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health Services, Human Services field, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services field, Social Work, Sociology, and Special Education

Preferences: Spanish fluency

License/Certifications: Valid Texas Driver’s License
