Job summary

TriStar Centennial Medical Center is seeking a Sepsis Nurse Coordinator to lead development and adherence to sepsis protocols within the facility. The role focuses on improving patient outcomes through collaboration with the medical staff and ongoing education for care teams, patients, and families.

The position requires TN RN licensure, a minimum of 3 years of relevant clinical experience, and familiarity with performance improvement tools. Sepsis experience is preferred and BLS is required.

Qualifications

• Associate’s Degree in Nursing or RN Diploma required.
• Bachelor’s Degree in healthcare related field preferred.
• Current RN licensure in Tennessee or compact state.
• 3 years of related clinical experience.
• Sepsis experience preferred.
• Knowledge of systems thinking and performance improvement tools; strong teaching skills.
• Current BLS certification.

Responsibilities

• Collaborate with medical and hospital staff to provide multidisciplinary care to patients with sepsis.
• Round with physicians to review clinical data.
• Provide education and clinical expertise to care team, patients, and families.
• Collaborate with physicians and staff to decrease mortality and length of stay.
• Identify patient learning needs and integrate into Plan of Care.
• Develop and implement individualized teaching plans for patients and families regarding sepsis diagnosis.
• Collect and analyze sepsis program patient data to assist with documentation and outcomes.
• Participate in rounds and respond to patient needs to improve outcomes.
