Qualifications

Bachelor’s degree in a related field with accreditation recognized by US DoE/CHEA or foreign equivalent.
Two (2) years of experience serving chronically homeless individuals with mental health and substance use disorders, including at least one year supervising others.
A combination of education and experience may be considered on a year-for-year basis; relevant experience required.
Proficiency with Excel, word processing, presentation, data management systems, and mobile application software.

Responsibilities

Provide direct case management to homeless Veterans with the goal of permanent housing, including assessments and service coordination.
Maintain caseloads for Veterans in Transitional Housing Program and develop personalized treatment plans.
Lead case conferencing and coordinated entry to identify affordable housing opportunities and communicate complex program issues clearly.
