Job summary

Salud Healthcare seeks a Provider Relations Manager to own the value-based care performance of a portfolio of physician practices across South Florida. You will be the strategic and operational partner from go-live, trusted to explain what is happening, why it matters, and what needs to change to improve patient engagement and outcomes.

Qualifications

• Minimum five years in provider relations, practice operations, or managed care.
• Three+ years focused on value-based care.
• Proven ability to translate data into actionable improvements.
• Strong ability to communicate with physicians and leaders.

Responsibilities

• Own end-to-end value-based care performance for assigned practices.
• Understand workflows, barriers, and operational needs.
• Diagnose performance gaps in HEDIS, Star, and visit completion.
• Implement workflows for annual wellness visits, care gaps, and risk opportunities.
• Lead performance reviews with physicians and leadership, set targets, track commitments.
