The Ohio State University Wexner Medical Center is seeking a Senior Pharmacy Technician (CPhT) to support Medication Management and Transitions of Care in the University Hospital Pharmacy Clinic. The role focuses on obtaining accurate admission medication histories, documenting allergies, and assisting with discharge medication processes under pharmacist supervision.

You will collaborate with care teams, communicate with patients and caregivers, and participate in ongoing training to maintain

Qualifications

• High school diploma or equivalent.
• Active OH Board of Pharmacy Certified Pharmacy Technician registration.
• PTCB/NHA certification.
• Three years of relevant health care experience as a Pharmacy Technician.
• Computer skills for data entry and records.
• Knowledge of medication names (brand and generic) and their use, doses, routes, and frequency.

Responsabilités

• Obtain complete admission medication history and allergy information.
• Document medication histories in EMR accurately.
• Assist with discharge medication process during inpatient or ambulatory encounters.
• Coordinate with pharmacist, physicians, and care team to resolve discrepancies.
• Facilitate medications delivery and access for patients and caregivers.
• Participate in ongoing training and maintain competency.

Formation

High school diploma or equivalent

OH Board of Pharmacy Certified Pharmacy Technician registration

PTCB/NHA certification

Three years of health care experience as Pharmacy Technician
