Job description

Scope of Position

The Medication Management / Transitions of Care Pharmacy Technician provides a valuable patient‑centered service by focusing on facilitating transitions of care, including obtaining and documenting a complete and accurate admission medication history and allergy information in the electronic medical record (EMR).

The TOC Pharmacy Technician completes the admission medication history during an inpatient admission, ambulatory encounter, pre‑admission testing, surgery encounter, or within a procedural area under the supervision of a pharmacist, physician, or advanced practice provider. Information is gathered from face‑to‑face or telehealth interviews, patient charts, and communication with patients, caregivers, physicians, retail pharmacies, and long‑term care facilities.

Throughout the hospital admission, the TOC Pharmacy Technician is responsible for collaborating with the patient and care team to facilitate discharge, which includes communicating options for discharge medications and long‑term medication delivery, obtaining credit card information, and facilitating Meds-to-Beds medication delivery when applicable. The technician also assists with accessing medication coverage and engaging the Medication Assistance Program or case management for prior authorizations, and ensures that the documented information is complete and accurate by correcting medication discrepancies or escalating to a pharmacist or provider for follow‑up.

Position Summary

The TOC Pharmacy Technician is responsible for obtaining a complete and accurate admission or ambulatory medication history and allergy information. The technician will also help to streamline and facilitate the discharge medication delivery process when applicable.

Minimum Qualifications
  • High School graduate or equivalent
  • Active OH Board of Pharmacy Certified Pharmacy Technician registration, PTCB/NHA certification
  • 2 years of relevant experience required; 4–6 years of relevant health care experience working as a Pharmacy Technician, with direct patient care preferred
  • Submit all required materials for registration as a Registered Pharmacy Technician to the OH Board of Pharmacy 10 business days before the official start date
  • Computer skills required in accordance with the requirements for data entry
  • Knowledge of medication names (both brand and generic) and their use, doses, routes of administration, and frequency at the time of hire or within ninety days of hire

Obtain PTCB/NHA certification and OH Board of Pharmacy Certified Registered Technician status within the first 270 days of employment and maintain for continued employment.
