Job summary
The Veterans Health Administration, Department of Veterans Affairs, is seeking a Senior Social Worker for the Home Based Primary Care (HBPC) program in the Veterans Health Care System of the Ozarks. Services cover a 50-mile radius around the HBPC clinic, delivering direct psychosocial care to adult and geriatric Veterans and their families.

Qualifications
- Psychosocial assessments to determine needs of Veterans and families.
- Discharge planning with other disciplines; ensure timely execution.
- Individual, family, and group counseling as appropriate.
- Education to Veterans and families on VA benefits and community resources.
- Provide consultation to social work interns on practice and policy.
Responsibilities
- Conducts psychosocial assessments to determine needs of Veterans and family members
- Active participant in treatment planning with other disciplines
- Discharge planning coordination and execution
- Provides counseling and education on resources and benefits
- Maintains data and supports staff training and community outreach
- Operates a government vehicle as required
