Job summary

The Home-Based Primary Care (HBPC) physician in the Central California Health Care System provides primary health care in the patient's home and in outpatient settings, collaborating with an interdisciplinary team to improve veterans' health and well-being.

The role includes assessment, diagnosis, treatment, preventive care, and coordination with nursing staff, with opportunities to participate in research activities and to educate patients and families.

Responsibilities

• assessment, diagnosis, treatment, and outcome measures of healthcare issues
• delivering preventive medical care including screening and vaccinations
• following up with additional home visits as appropriate
• collaborating with nursing personnel in the care of patients
• providing medical care for patients with acute and chronic health problems
