Job summary

U.S. Department of Veterans Affairs is seeking a Senior Social Worker to serve as the Health Care System's Liaison/Case Manager for the Grant & Per Diem and HCHV programs in Homeless Programs Service. You will oversee community-based Veterans programs, monitor clinical care, compliance, and fiscal administration for grant recipients.

The role requires a CSWE-accredited MSW, state licensure, and the ability to supervise licensure. Strong coordination with teams and community partners is essential.

Qualifications

• Master's degree in social work from a CSWE-accredited school is required.
• Advanced licensure/certification at the state level is required or completed within grandfathering provisions.
• Ability to supervise licensure and provide clinical supervision as needed.
• Knowledge of ethics, compliance, and program administration in VA settings.
• Experience with GPD/HCHV programs is preferred.

Responsibilities

• Serve as liaison and case manager for GPD and HCHV programs.
• Coordinate interdisciplinary team and conduct site inspections.
• Monitor clinical care and fiscal compliance of grant-funded programs.
• Maintain accurate billing, admissions, and discharge records.
• Provide direct case management and advocacy for Veterans.
• Coordinate mental health, substance use, and primary care services across care episodes.
• Engage community resources and ensure VA benefits information is shared.
