Job summary

Qualifications

Responsibilities

• Serve as the primary liaison for Contract Residential Services (CRS).
• Coordinate admissions, placements, transitions, referrals, and follow-up for Veterans.
• Provide advanced clinical social work services including case management and care coordination.
• Monitor performance, ensure compliance, and maintain VA records.
