U.S. Department of Veterans Affairs' Central Alabama VA Healthcare System is seeking a licensed Social Worker to serve as Health Care for Homeless Veterans (HCHV) Coordinator.

The role leads program management, outreach, and coordination of care for homeless veterans with psychiatric and substance use disorders to promote health and social functioning. The Coordinator develops housing partnerships, ensures policy compliance, mentors staff, and represents HCHV within the VA and the community,

Competenze

• Must be a licensed independent Social Work practitioner.
• Experience coordinating homeless services or HCHV/VA programs preferred.

Mansioni

• Coordinate the HCHV program and oversee outreach and coordination of care.
• Provide functional leadership and coordination of HCHV staff.
• Develop, coordinate, and evaluate community residences in the catchment area.
• Ensure compliance with VA policy, CARF standards, and center procedures.
• Monitor the program's data and prepare reports on outcomes and activities.
• Represent the HCHV program within the VA Medical Center and the local community.
