Job summary

• Provide counseling to support adjustment to dialysis and treatment goals.
• Educate staff on psychosocial impact of chronic kidney disease.
• Facilitate transplant referral processes and access to resources.
• Coordinate with insurance and community resources to support patients.
• Document interventions and participate in quality improvement activities.
