• Maryland LMSW licensure required.

Responsabilités

• Complete psychosocial assessments and plan care accordingly.
• Autonomously propose and execute safe discharge plans with the care team.
• Provide crisis intervention and trauma-informed care as needed.
• Evaluate response to interventions and participate in quality improvement.
• Share resources with patients and families; maintain referral lists.
• Coordinate referrals and inform care team of updates.
• Facilitate individual, family, and group counseling using brief therapy models.
• Coordinate with outside agents while protecting HIPAA and Releases of Information.
• Conduct patient education sessions and train SW I & II staff.
• Collaborate with departments to improve equitable patient-centered care.
• Identify systemic barriers and contribute to equity initiatives.
• Perform departmental administrative duties as assigned.
• Complete all department tasks assigned by supervisor.
