Job summary

The Quarters at Des Peres is seeking a Social Services Supervisor to lead discharge planning and psychosocial support for residents in a compassionate, team-oriented environment.

The role requires collaboration with care teams, strong communication skills, and a commitment to resident rights and safety. A social work degree and licensure are preferred, with relevant healthcare experience and a focus on ethical practice expected.

Qualifications

• Bachelor’s degree in social work preferred or related field including sociology, special education, rehabilitation counseling, psychology
• One year of supervised social work experience in a health care setting preferred
• Current State Clinical Social Work Licensure
• Ability to work cooperatively as a member of a team
• Ability to communicate effectively with residents and their family members at all levels
• Ability to react decisively and quickly in emergency situations
• Ability to maintain confidentiality
• Ability to read, write and speak English
• Knowledge of training techniques for clinical staff
• Good ethical practice
• Relationship management

Responsibilities

• Coordinate and integrate social service/discharge planning activities within the hospital system through effective communication at all levels
• Develop specific, measurable, realistic plans and objectives to enhance social service daily
• Perform psychosocial assessment and reassessment on all residents and address needs of residents and families
• Provide resident care in a safe environment and reduce health care errors
• Form teaching plans based on identified learning needs and involve family
• Promote residents’ rights and interpret the Residents’ Bill of Rights
• Interact professionally with resident and family to involve them in care plans
• Provide individual and group support to residents and families during adjustment or crises
• Direct Social Service/Discharge Planning to meet resident needs and comply with center policy
• Provide resources and support to Social Service/Discharge Planning Staff/Clinical Staff
• Maintain a culture of safety and quality
• Collaborate with medical staff and departments to maintain good rapport
