Job summary

The Quarters at Des Peres is seeking a Social Services Supervisor to coordinate discharge planning and psychosocial assessments across our resident population. You will work with residents, families, and clinical staff to ensure safe, dignified care and adherence to the Residents’ Bill of Rights.

Ideal candidates hold a social work degree (BSW/MSW), a current state licensure, and prior healthcare social work experience.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Current State Clinical Social Work Licensure required.
• 1 year of supervised social work experience in a healthcare setting preferred.
• Ability to work cooperatively as part of a team.
• Ability to communicate effectively with residents and families at all levels.
• Ability to react decisively in emergencies.
• Maintain confidentiality and ethics in practice.

Responsibilities

• Coordinate social service/discharge planning within the hospital system through effective communication at all levels.
• Develop specific, measurable plans to enhance social service daily operations.
• Perform psychosocial assessments and reassess residents and families’ needs.
