Job summary

National Health Foundation is seeking an Associate Director of Social Services to provide operational leadership, oversee discharge planning, and ensure high-quality client-centered services across Recuperative Care locations.

The role requires strong leadership, sound judgment, and the ability to balance priorities while working with diverse teams and community partners to support housing stability and program compliance.

Qualifications

• Bachelor’s degree required; Master’s preferred for advanced practice and leadership roles.
• Minimum five years in social services, healthcare, or related field.
• Minimum three years of supervisory or management experience.
• Experience with homelessness, behavioral health, and housing instability preferred.

Responsibilities

• Lead the Social Services department and oversee staff performance.
• Oversee discharge planning, case management, and client services.
• Ensure documentation accuracy and quality assurance across records.
• Ensure program compliance with contracts, grants, and regulations.
• Develop partnerships with hospitals, housing providers, and community resources.
• Support program development and staff training.
