Job summary

The Orchards is a health center offering assisted living, skilled nursing and memory care. We are seeking a Social Services Director (SNF Post Acute Case Manager) who serves as the licensed Social Worker for the Health Center and ensures residents' emotional and social needs are met.

You will oversee transitions, coordinate care, and develop social histories from admission through discharge, with a salary of $80,000–$85,000 and a 15% bonus target.

Qualifications

• Social Services Degree with 5+ years of experience or LVN with 5+ years of experience.
• Experience with Title 22 and OBRA Regulations including Medicare, PPS, MDS, Care Planning and HIPPA.
• Ability to multi-task and prioritize effectively, and work closely with residents, families and staff.

Responsibilities

• Oversee SNF stay with focus on patient experience, length of stay and transition planning.
• Coordinate transitions and care efficiencies for SNF residents (short- and long-term).
• Develop social history and assessment for care plan, noting preferences and needs.
• Assist residents/families with transition, orientation to facility and rights.
• Meet with patients/families to evaluate needs and readiness for transition.
• Contribute to care plan development and link to additional resources.
• Maintain communication with SNF staff, physicians, and other disciplines for transition plans.
• Facilitate meetings related to antipsychotics, dose reduction, and psychosocial services.
• Participate in policy reviews and quality assurance meetings.
• Ensure Advanced Directives are complete and resident charts updated.
