Job summary

The Town of Yarmouth Massachusetts seeks a Licensed Social Worker to join the Senior Services team. You will provide direct services to homebound elders, coordinate with regional agencies, and lead training for staff and volunteers.

Requires a Master’s in social work and an LCSW, plus minimum three years’ experience in social services; Massachusetts Class D license and CORI background check are required.

Qualifications

  • Master’s degree in social work (MSW) required.
  • Licensed Clinical Social Worker (LCSW) required.
  • Three years of progressively responsible experience in social services or equivalent combination of education and experience.

  • Conducts home visits with client to establish trust and develop action plan.
  • Connects senior with solutions promoting community, regional and countrywide resources.
  • Utilize efficient and effective time management tools.
  • Maintain casework files per standard methods to protect confidential information in locked cabinet and/or secure computer folder system.
  • Develop data collection forms and methods. Report all home visits, meetings, phone calls, training sessions, committee meetings and events in monthly metric format.

The Town of Yarmouth is searching for a Licensed Social Worker to join their Senior Services team. This position works under the general direction of the Director of Senior Services.

Creates, develops, administers, and coordinates services and programs to address the needs of home bound elders and those elders suffering from mental health challenges.

Responsible for client related crisis intervention, interface with other Town departments, and maintaining a safe environment relative to client mental health issues.

Conducts home visits with client to establish trust and develop action plan. Connects senior with solutions promoting community, regional and countrywide resources.

Utilize efficient and effective time management tools.

Maintain casework files per standard methods to protect confidential information in locked cabinet and/or secure computer folder system.

Develop data collection forms and methods. Report all home visits, meetings, phone calls, training sessions, committee meetings and events in monthly metric format. Summary reports as required.

Provide training (in-service) to Senior Services staff, leading internal professional development and sessions on mental health and homebound seniors.

Train and supervise volunteers needed for maximizing Outreach capacity to elders through Yarmouth Senior Services.

Foster and maintain relationships with external organizations and government agencies to improve delivery of critical services to elders. These agencies include but are not limited to the Executive Office of Elder Affairs, Massachusetts Association of Councils on Aging, Elder Services of Cape Cod and the Islands, National Council on Aging, COAST Outreach, Arbor, Gosnold, Home Care Agencies, Healthcare, etc.

Meet monthly with the COAST Outreach and liaison committees such as Wanderers, Elder Abuse, ADA, Hoarding, etc.

Participates in local and regional efforts to expand and improve services to elders.

Prepare Outreach topic text for monthly bulletin published by the Division of Senior Services.

Work cooperatively with other town departments to assure effective, efficient resource and solution information dissemination and connection to Elders.

Performs similar or related work as required, directed or situation dictates.

Physical Requirements

Requires ability to walk and drive to access private homes; vision correctable to normal limits; must be able to operate computer and motor vehicle. Minimal physical effort generally required for work performed in office.

SPECIAL REQUIREMENTS
  • Massachusetts Class D Driver's license
  • CORI background check required as condition of employment
