• Ability to relate to and communicate with older adults who are dependent on community services to remain in their own homes.

Responsibilities

• Coordinates Friendly Visitor Program.
• Telephones and conducts in-home visits for a caseload of up to 36 older adults.
• Serves as a certified SHICK counselor and assists older adults with information for insurance decision-making.
• All other duties as assigned.
