Job summary

Leoforce is seeking an experienced Registered Nurse (Long Term Care) in Acworth, GA for a permanent, full-time role. The ideal candidate will have 5+ years of nursing experience and a current Georgia RN license, with a patient-centered approach to long-term care.

Responsibilities include comprehensive assessments, care planning, medication administration, and collaboration with the care team to support residents’ health, dignity, and quality of life in a dedicated facility.

Qualifications

• Minimum of 5 years of nursing experience.
• Current, active Georgia Registered Nurse license or valid multistate RN license for Georgia.
• Graduate of an accredited nursing program.
• Strong assessment, clinical decision-making, medication administration, and care-planning skills.
• Knowledge of long-term care practices and regulatory requirements.

Responsibilities

• Provide compassionate, evidence-based nursing care tailored to residents' medical, physical, and emotional needs.
• Complete comprehensive assessments, monitor changes in condition, and communicate concerns to the care team.
• Develop, implement, and evaluate individualized care plans with residents and families.
• Administer medications and treatments safely, documenting responses.
• Support chronic disease management, wound care, infection prevention, fall prevention, and other long-term care services.
• Maintain accurate clinical documentation per policies and regulations.
• Educate residents and families about health conditions and preventive care.
• Guide and support nursing assistants and other care team members within scope of practice.
• Respond to urgent situations and ensure safe care transitions.
• Champion resident rights, safety, dignity, and continuous quality improvement.
