Job summary
21st Century Home Health Services in the Bay Area is seeking a Registered Nurse, Level 2 Home Health Case Manager to lead care for patients in Novato and surrounding areas. You will oversee 2 Nurses, develop and execute plans of care, and coordinate with physicians and the interdisciplinary team to prevent rehospitalizations.
Ideal candidates have 1+ year in home health case management, CA RN license, BLS/CPR, and a drive for compassionate, autonomous leadership.
Qualifications
- One year of home health case management experience preferred.
- Current, unrestricted CA RN license required.
- BLS/CPR certification required.
- Experience supervising Nurses is highly preferred.
Responsibilities
- Create, implement and manage the plan of care to promote wellness at home.
- Oversee 2 Nurses responsible for executing the care plan.
- Conduct Start of Care, Evaluations, and discharge planning.
- Collaborate with physicians and interdisciplinary team to deliver best care.
