Sharp HealthCare in San Diego, California, is seeking an RN CM II to assess, coordinate and monitor patient care in inpatient and ED settings. You will collaborate with physicians, social workers and the interdisciplinary team to align clinical needs with utilization and regulatory requirements.

The role requires a BSN, 2 years of recent clinical experience, and 3 years in acute care or case management; Masters and CCM/ACM preferred.

Qualifications

• Bachelor's Degree in Nursing or equivalent degree.
• 2 years' recent pertinent clinical experience as defined by the CBA.
• 3 years acute care nursing experience or case management experience.
• 3 years case management, utilization review, care coordination experience.

Responsabilités

• Assess, develop, implement and monitor a comprehensive plan of care for each patient/family in collaboration with the physician, social worker and interdisciplinary team.
• Collaborate with physicians, social workers and the broader care team to ensure appropriate setting and level of care while balancing clinical needs with utilization review demands.
• Mentor and precept new hires (CMI, SW, LVN Care Coordinators and CRCs).
• Lead, coordinate and contribute to discharge planning and care coordination initiatives.
