Job summary

Jobtailor is seeking an experienced RN capable of managing care for members with complex needs in Colorado. The role involves assessing patients, developing individualized plans, and coordinating authorizations and referrals to ensure timely access to services.

Ideal candidates will have 5+ years of clinical experience, a current RN license (multi-state licensure preferred), and demonstrated case management proficiency. Telephonic communication and strong problem-solving skills are essential.

Qualifications

• BA/BS in a health-related field with 5+ years clinical experience or equivalent combination.
• Current, unrestricted RN license in applicable state(s) required with multi-state licensure.
• Case management experience preferred and CM certification preferred.

Responsibilities

• Manage care for members with complex and chronic care needs within licensure scope.
• Assess members and develop individualized care management plans.
• Implement care plans via authorizations and referrals.
• Coordinate internal and external resources to meet needs.
• Monitor and evaluate care plan effectiveness and modify as needed.
• Interface with Medical Directors and Physician Advisors on treatment plans.
