Job summary

City of Springfield, MA is seeking a responsible, full-time Elder Affairs outreach worker to provide case management and assistance to seniors 60+ and their caregivers in Springfield. You will access Medicare, Medicaid, fuel assistance, and other services, under the Director of Elder Affairs.

Responsibilities include developing care plans, coordinating programs, maintaining confidential files, and advocating for seniors both at home and in the community.

Qualifications

• Bachelor's Degree in Social Work or related field; 1 year experience required, or 5 years of equivalent experience.
• Bilingual preferred.

Responsibilities

• Provides case management services and application assistance to seniors 60+ and caregivers.
• Develops care plans and makes referrals; maintains intake, assessment, progress notes, and client authorization forms.
• Advocates for seniors' rights and entitlements; may attend court cases with no family present.
