Job summary

Stonehaven Senior Living in Eagan, MN seeks a Resident Care Coordinator (RCC) to partner with the Director of Nursing and oversee resident care services in line with federal/state/local standards. The role emphasizes planning, organizing, and directing nursing functions across facilities, ensuring quality care and regulatory compliance.

Responsibilities include coordinating staffing, policy development, care plans, and collaboration with health care providers.

Qualifications

• Minimum of two (2) years of nursing experience.
• Leadership or care coordination experience preferred.
• Current, active nursing license in the applicable state, in good standing, required.

Responsibilities

• Assist to develop, maintain, update, communicate and enforce facility policies and procedures for nursing services.
• Coordinate nursing service schedules and communication with other services to ensure continuum of care.
• Participate in budget and staffing planning for the nursing service department.
• Admit, transfer, and discharge residents with care documentation and appropriate approvals.
• Ensure compliance with federal, state, and local regulations and participate in quality improvement programs.
