Job summary

Legacy Healthcare Services, Inc. is seeking a dedicated Physical Therapist to join our senior living rehabilitation team.

You will provide rehabilitative services to residents under medical orders and develop initial and subsequent treatment programs based on clinical findings. You will treat residents one-on-one, perform private sessions with a functional-based approach, and participate in clinical conferences to coordinate the plan of care.

Qualifications

• Degree in the field of Physical Therapy.
• Current Physical Therapy license in state of practice or ability to obtain prior to employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, with individualized attention through private treatment sessions.
• You have the ability to individualize treatment sessions.
