Job summary

Legacy Healthcare Services is seeking a Physical Therapist to provide rehabilitative care to senior residents in our communities. You will plan and implement treatment programs, work with a multidisciplinary team, and ensure individualized sessions to improve quality of life.

Responsibilities include conducting private, one-on-one sessions, participating in clinical conferences, and adapting plans to patient progress in a supportive, not-for-profit environment.

Qualifications

• Degree in area of discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group setting, so each patient gets individualized attention through private treatment sessions with a functional based treatment approach.
• You have the ability to individualize treatment sessions.
