Job summary

Tender Touch Rehab Services in New Jersey is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team on-site, providing individualized therapy to seniors in skilled nursing and long-term care settings.

You will implement PT treatment plans focused on improving mobility, safety and quality of life, including therapeutic exercise, gait and balance training, functional activities, EMR documentation, and education for patients and caregivers.

Qualifications

• Certified Physical Therapist Assistant credentials.
• Experience in geriatrics or skilled nursing settings is advantageous.
• Ability to work on-site in a team-oriented environment.

Responsibilities

• Implement PT treatment plans to improve mobility and function.
• Lead therapeutic exercise and gait/balance training.
• Perform functional activities and patient care education.
• Accurate EMR documentation of patient progress.
• Collaborate with an interdisciplinary team to ensure quality care.
