Qualifications

• Requires bachelor’s degree in social services, addiction, mental health or closely related health services field with 3+ years of post-Bachelor degree professional experience in the addictions or counseling field
• OR master’s degree in social services, addictions, mental health or closely related health services field with 1+ years of post-Masters degree professional experience in the addictions, therapy or counseling field
• Degree must be from an accredited college or university.

Responsibilities

• Conduct client assessments to identify history, current presentation, problem areas, and ongoing clinical need for treatment and risk assessment.
• Perform ongoing assessments of chemical dependence services required by the program.
• Prepare and carry out initial treatment planning and progress reviews, documenting as required.
• Lead group, individual, or family sessions using evidence-based treatment modalities and document outcomes.
• Provide client and family education related to substance abuse/psychiatric issues.
• Ensure timely, clear documentation of client care, treatment and incidents.
• Provide crisis intervention and counseling as needed.
• Plan and document aftercare for assigned clients.
• Serve as liaison with insurance carriers and referral sources as needed.
• Provide case management to ensure individualized quality care.
• Conduct educational workshops within the hospital and community.
• Coordinate with self-help groups and EAP professionals.
• Evaluate client needs to determine appropriate level of care and referrals.
