Job summary

CenterWell Primary Care, part of Humana, seeks a Primary Care Physician to provide outpatient care for adults and seniors in a collaborative, patient-centered environment. The role emphasizes value-based care, with opportunities for professional growth and leadership.

The position requires an MD/DO, an active license, board certification in Family, Internal, or Geriatric Medicine, and at least 2 years' experience in geriatric care. Compensation includes benefits and relocation support.

Qualifications

• MD or DO credential from an accredited medical school.
• Active, unrestricted medical license in the practicing state.
• Board certification in Family Medicine, Internal Medicine, or Geriatrics.
• Experience with value-based care or high-acuity geriatric patients.
• TB screening will be conducted if hired.

Responsibilities

• Deliver outpatient care to adult and geriatric patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and consult with specialists.
• Participate in quality improvement projects.
• Collaborate with interdisciplinary teams to ensure holistic care.
• Contribute to innovation in care delivery.

Education

MD or DO from an accredited medical school
