CenterWell Senior Primary Care, a Humana subsidiary, seeks a Primary Care Physician for an outpatient clinic in Homeland Park, SC. This full-time, onsite role reports to the Associate Medical Director and emphasizes building lasting patient relationships in a value-based setting.

You will manage chronic conditions, coordinate with specialists, and document care efficiently. A SC medical license and DEA are required, with a strong focus on preventive and comprehensive care.

Qualifications

• MD or DO from an accredited medical school or international medical graduate with ECFMG certification and US residency.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active, unrestricted medical license in South Carolina or ability to obtain licensure.
• Active, unrestricted DEA registration or ability to obtain prior to start date.
• 2+ years of experience caring for senior populations within Medicare Advantage or value-based care settings.
• Experience supervising Advanced Practice Providers (NPs/PAs).

Responsabilités

• Provide comprehensive primary care services to a panel of adult and senior patients.
• Care for patients with chronic and complex medical conditions.
• See 12-15 patients per day with longer visits for stronger relationships.
• Manage longitudinal care plans focused on prevention, wellness, and chronic disease management.
• Complete accurate, same-day documentation to support outcomes.
• Manage daily clinical inbox: lab reviews, patient messages, prescriptions, follow-up care.
• Coordinate care with specialists, hospitals, community resources and care team.
• Apply Medicare risk adjustment and annual condition documentation practices.

Active SC medical license
