Job summary

Humana Inc. is recruiting for an on-site, patient-facing physician within CenterWell Senior Primary Care in Greer, SC.

You will report to the Associate Medical Director and provide in-visit care as well as ongoing patient management in a value-based model. You will see 12-15 patients per day with longer visits, manage complex chronic conditions, complete same-day documentation, and coordinate care with the care team to support better outcomes.

Qualifications

• MD or DO from an accredited program.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Active, unrestricted medical license in South Carolina.

Responsibilities

• See 12-15 patients per day with longer visits.
• Manage patients with complex and chronic conditions.
• Complete accurate, same-day documentation to support patient care and outcomes.
• Manage a daily clinical inbox (labs, messages, follow-ups).
• Coordinate care with specialists and care team members.
• Apply Medicare risk adjustment principles through documentation.
