Job summary

CenterWell Senior Primary Care in Seneca, South Carolina, is seeking a Primary Care Physician for our outpatient clinic. This is a full-time, onsite role focused on adult and senior patients within a value-based care model, reporting to the Associate Medical Director.

Qualifications

• MD or DO from an accredited medical school or graduate of an international medical school with ECFMG certification and U.S. residency.
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active, unrestricted medical license in South Carolina or ability to obtain licensure.
• Active, unrestricted DEA registration or ability to obtain before start date.

Responsibilities

• Provide comprehensive primary care to adults and seniors.
• Care for patients with chronic and complex conditions.
• See 12–15 patients per day with longer visits for strong relationships.
• Manage longitudinal care plans focused on prevention, wellness and chronic disease.
• Complete accurate, same-day documentation to support quality outcomes.
• Manage daily clinical inbox: lab reviews, messages, prescriptions, follow-up care.
• Coordinate care with specialists, hospitals, community resources and care team.
• Apply Medicare risk adjustment and annual condition documentation practices.

Education

MD or DO from an accredited medical school

ECFMG certification + U.S. residency

Board Certified or Board Eligible in Family/Internal/Geriatric Medicine

Active SC medical license or eligible
