Job summary

CenterWell Senior Primary Care in Greer, South Carolina, is seeking a Primary Care Physician for our outpatient clinic. This full-time, onsite role focuses on building meaningful patient relationships while delivering high-quality, value-based care under Humana.

You will manage a panel of adult and senior patients, coordinate care with specialists, and document encounters accurately to support outcomes. Requirements include MD or DO, SC medical license, and Board Certification or eligibility in

Qualifications

• MD or DO from an accredited medical school or international medical graduate with ECFMG certification and completion of an accredited U.S. residency program.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Active, unrestricted medical license in South Carolina or ability to obtain licensure.
• Active, unrestricted DEA registration or ability to obtain before start date.

Responsibilities

• Provide comprehensive primary care services to a panel of adult and senior patients.
• Care for patients with chronic and complex medical conditions.
• See 12-15 patients per day with longer visits to foster strong patient relationships.
• Manage longitudinal care plans focused on prevention, wellness, and chronic disease management.
• Complete accurate, same-day documentation to support quality outcomes and patient care.
• Manage a daily clinical inbox including lab results, patient messages, prescriptions, and follow-up care.
• Coordinate care with specialists, hospitals, community resources, and care team members.
• Apply Medicare risk adjustment and annual condition documentation practices.
