Job summary

Millennium Physician Group in Vero Beach, FL seeks a Physician, General / Primary Care to provide comprehensive medical services across the lifespan in our ambulatory practice. You will evaluate, diagnose, and manage acute and chronic conditions, order tests, and develop evidence-based treatment plans while coordinating care with APPs and the care team.

The role may include Associate Medical Director responsibilities and chart audits to support quality care.

Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care using history, physical exams, and evidence-based medical decision-making.
• Develop and oversee individualized treatment plans including medications, preventive services, immunizations, and chronic disease management.
• Diagnose and manage a broad range of primary care conditions using current clinical guidelines.
• Order, interpret, and integrate labs, imaging, and diagnostics to guide treatment.
• Perform office-based procedures aligned with credentialing and scope.
• Conduct annual wellness visits, preventive screenings, and chronic care management to improve outcomes.
• Coordinate care with specialists and community resources; determine need for specialty care.
• Collaborate with APPs, nurses, pharmacists, and other professionals to coordinate care.
• Educate patients and caregivers on diagnoses, medications, and preventive health.
• Document encounters and outcomes accurately in the EHR while complying with standards.
• Participate in quality improvement, peer review, and population health initiatives.
