Job summary

CenterWell Senior Primary Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative care team focused on seniors. You will provide comprehensive, value-based care and collaborate with physicians, pharmacists, and care coordinators in an on-site outpatient clinic.

The role emphasizes team-based, patient-centered care with longer visits, on-call rotation, and coordination of referrals, hospital/SNF liaison, DME and home health services.

Qualifications

• Master’s Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening will be conducted if selected.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Demonstrate experience delivering value-based care.
