Job summary

CenterWell Senior Primary Care in Florida is hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative, multidisciplinary team at our CenterWell The Villages clinic. You will deliver personalized, high-quality care alongside physicians, pharmacists, care coaches, and behavioral health specialists, with an emphasis on the whole-patient approach and value-based care models.

On-site role with a competitive pay range of $123,800 to $170,300 per year, plus a bonus plan

Qualifications

• Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification.
• Active, unrestricted NP or PA license in the state of Florida.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
• One +years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions.
• Proficiency using an EMR.
• TB screening required if selected for this role.
