Job summary

CenterWell Senior Primary Care in Louisiana is seeking a Nurse Practitioner or Physician Assistant to join our in-person, team-based care model serving seniors. You’ll provide comprehensive, value-based care following clinical standards and collaborate daily with a care team across physicians, pharmacists and care coaches to deliver high-quality outcomes.

You’ll screen for TB if selected, manage referrals and care coordination for 3 outpatient clinics in the area, and participate in on-call

Qualifications

• Master's Degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening may be required as part of the role.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Have experience providing value-based care.
