Job summary

CenterWell Senior Primary Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality, senior-focused primary care in a collaborative team setting.

This in-person role supports 3 outpatient clinics, with TB screening required, and reports to the Area Medical Director. Competitive compensation ranges from $123,800 to $170,300 annually, plus bonus eligibility and comprehensive benefits.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in state of practice.
• TB screening may be required.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Provide value-based care.
