Job summary

CenterWell Senior Primary Care provides proactive, preventive care to seniors, delivering personalized experiences and spending more time with patients to improve outcomes. The role supports a value-based care model within Humana’s Primary Care Organization, with a clear focus on coordinating care across teams.

The position offers a 40-hour work week, competitive benefits, and a compensation range of $123,800–$170,300 per year, with bonus incentives based on performance.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Experience in value-based care, primary care or Medicare Advantage settings.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Provide value-based care.
