CenterWell Senior Primary Care in Louisiana seeks a Nurse Practitioner or Physician Assistant to join a collaborative, in-person team delivering high-quality senior care. You will provide comprehensive care, collaborate daily with the care team, and coordinate referrals, hospital/SNF transitions, DME and home health services.

Requirements include a Master's in Nursing or PA certification and active NP/PA licensure. TB screening is required.

Your Role & Responsibilities

Additional Information

• You’ll report to the Area Medical Director.
• This is an in-person role.
• You will support 3 of our outpatient clinics in the area.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
